This year the Wellness Team at South Sanpete School District will be offering a
School Based Mental Health Screening on February 9, 2026 from 11 am - 6 pm at
the District Office for Middle School and High School students. Screening involves
your child completing the PHQ9 and GAD7 Mental Health Screening Assessment
forms with the help of the Wellness Team. Screening helps us understand student
strengths, identify areas of need, and generate ways to support their success at
school.
Please note that results from all screeners will NOT be used to label, diagnose,
categorize, or provide treatment to your child. The screener will be used to gather
more information about their needs. The Mental Health Screening will only be
given with parent/guardian permission. If the results from the screeners indicate
concerns, we will contact you to discuss results and recommendations for
school-based interventions.
Both the GAD7 and PHQ9 Mental Health Screeners are attached to this letter for you
to preview. Please read through the screener and reach out to us with any questions.
If you would like your child screened, please bring them to the District Office at
39 South Main Street in Manti on Monday, February 9th between 11 am and 6 pm.
No appointment necessary.

CCHC Therapy and Coaching Service and Coaching services for parents, student, families, faculty and staff. This therapy support is unlimited and allows for one meeting a week for 30 minutes per session. The therapy can happen for any reason. Here are some examples, but not limited to:
- How to parent and help my student because of divorce
- Staff member going through divorce and needs support
- Loss of a family member, parent, spouse, student, etc
- Anxiety
- Suicidal thoughts, suicide, etc
- Eating disorders
- Dispute/conflict
- Anger
- Depression
- Behavior
- Health or related decline in abilities
- If the concern is beyond what the therapist is able to provide they will recommend help beyond what they can provide
- Etc
