1. PURPOSE

The benefits of the SEP/SEOP include:

a. Assisting students to acquire knowledge to make reliable, useful and timely data on student progress       available to parents, educators and students.

b. Helping students acquire knowledge and skills to enable them to take responsibility for their decisions and make appropriate choices.

2. POLICY

It is the policy of the South Sanpete School District to develop a personalized Student Education Plan (SEP) or Student Education Occupational Plan (SEOP) for each student which shall:

a. Recognize the student's accomplishments and strengths.
b. Focus on education and career development goals and progress towards meeting student achievement standards as defined in UPASS.
c. Make use of relevant information and data on student progress.
d. Incorporate information gained from UPASS testing which will be shared with parents.

3. PROCEDURE

Elementary School Conference (K-6)

a. Hold two annual individual SEP conferences for each student in grades K-6 involving student, parent or guardian, and school personnel.
b. Provide training to teachers on their role in assisting in the development and implementation of SEP's.
c. Each elementary school SEP shall contain:

1. Relevant data about the student's progress, accomplishments, UPASS test data, strengths, and areas needing attention.

2. Evidence of holding at least two annual conferences involving the student, parent or guardians and school personnel.

Middle School Conference (7-8)

Seventh Grade SEOP

a. SEOP's shall be coordinated by the counselor.
b. Guidance Curriculum activities will be taught in conjunction with the TLC (Technology, Life and Career) classes, in the areas of Self Knowledge, Education and Occupation Exploration, and may include career fair guest speakers, field trips, choices futures, etc.
c. Students shall participate in small groups (classroom size).
d. The counselor shall record evidence of parent/guardian involvement in the SEOP process.
e. There will be an educational document completed and signed at the conclusion of the activities.

Eighth Grade SEOP

a. Annually hold one individual and one small group (classroom size) Counselor Coordinated SEOP conference with students.
b. Parent/guardians are required to attend the individual conference.
c. Provide training to teachers on their role in assisting in the development and implementation of SEOP's.
d. Each 8th grade SEOP shall contain:

1. An opportunity to celebrate the student;

2. Time to discuss student progress in the classroom across all disciplines, reviewing test data, grades and study habits;

3. Set expectations for future progress;

4. Evidence of post-secondary educational and occupational planning;

5. Registration for 9th grade high school classes; and

6. Evidence of parent/guardian involvement in the SEOP process.

High School Conference (9-12)

a. Annually hold one individual and one small group (classroom size) SEOP Conference with the student. Parents/guardians will be invited each year and will be required to attend 9th and 11th grade years.
b. Provide training to teachers on their role in assisting in the development and implementation of SEOP's.
c. Each high school SEOP shall contain:

1. Relevant data about the student's progress towards graduation, U-PASS data, accomplishments, and                
    strengths.
2. Evidence of a 4-year plan including course selection appropriate to career goals.
3. Evidence of post-secondary educational and occupational planning.
4. Evidence of parental involvement in the SEOP process.
5. Where applicable, indication of plans for early graduation.
