School to Post-School Transition

Transition Assessments are different assessments that can be used to help determine preferences and prepare for life goals pertaining to independent living, educational planning, and career planning.  Students who are highly involved in their own planning processes are more likely to be successful in achieving their desired goals.

From 2012 to 2014, South Sanpete School District Special Education teachers in the middle and high schools participated with the National Secondary Transition Technical Assistance Center (now NTACT and the Utah State Office of Education's Special Education Transition personnel in building our capabilities to assess and teach students in School to Post-School Readiness.

Transition planning with students with disabilities begins with comprehensive transition assessments to help students determine postsecondary goals based on their preferences and interests. This document contains a description of the transition assessment process, as well as links to transition assessments and curricula to help schools prepare students for adult life.

Casey Life Skills

 

 

 

 

 

 

 

 

Casey Life Skills (CLS) is a free tool that assesses the behaviors and competencies youth need to achieve their long-term goals. It aims to set youth on their way toward developing healthy, productive lives. Examples of the life skills CLS helps youth self-evaluate include:

Maintaining healthy relationships

Work and study habits

Planning and goal-setting

Using community resources

Daily living activities

Budgeting and paying bills

Computer literacy

Their permanent connections to caring adults

 Self-Determination

 

The ChoiceMaker Self-Determination Assessment   a curriculum-referenced tool that measures students' self-determination skills and progress in the ChoiceMaker Self-Determination Curriculum. Educators complete a 5-point Likert scale response for each of the 62 items across the student skills and opportunities at school sections.   The AIR Self-Determination Assessment focuses on two main components: Capacity and Opportunity.  Capacity refers to the knowledge, abilities, and perceptions which allow the student to be self-determined.  Opportunity refers to the student's chances to use their knowledge and abilities.  The AIR Self-Determination Scale is available free of charge in a variety of forms:  student form,  (Spanish) student form, parent form, and educator form. (from http://www.ou.edu/content/education/centers-and-partnerships/zarrow/self-determination-assessment-tools.html)
  The ARC Self-Determination Scale was developed by Dr. Michael Wehmeyer and colleagues to  measure strengths and weaknesses of adolescents with disabilities, facilitate student involvement in educational planning and instruction to promote self-determination as an educational outcome, and assess student self-determination skills for research purposes. Career Road Map: The Career Road Map provides examples of typical steps or phases for getting started in a specific occupation. The map begins with 7th grade courses and ends with Utah employers most likely to hire for that occupation and/or actual Utah job openings. You can gain a basic understanding of the standard education, training, license, certification, degree, etc. requirements for occupations.
